Payment for Services and Insurance Reimbursement

We accept insurance and are pleased to be able to offer in-network services with most major insurance companies. In-network status varies by provider.  We recommend that you consult your insurance company about potential pre-approval requirements and reimbursement issues prior to your appointment with us. Your insurance plan may cover all or part of the cost of services.  Click here to download and print an evaluation referral form for your doctor to complete prior to your visit.

We are also happy to work with individuals who do not have insurance, on a self-pay basis. 

Co-pays, balances, and payment for non-covered services and products are due the day of service. 

We accept:

• Cash (Please note: We do not keep cash on the premises and are not able to make change)
• Check (made out to Complete Neuropsychology Services) 
• Debit, or credit cards (Visa, MasterCard, Discover, American Express) 
• HSA-managed credit cards.  
• You can also pay online via SQUARE invoice emailed to you (we use a company called SQUARE for credit card transactions).  

24-hour notice is required for cancellation, except in emergency situations.  We charge a $45 fee for late cancellation and no-shows. 

It is the client’s responsibility to consult their insurance company prior to the initial visit to determine coverage.  

**Fees for service are determined by Complete Neuropsychology Services administrative staff, who reserve the right to change rates and policies at any time. Further,  if you do not pay for services as previously agreed upon, the clinic reserves the right to attempt to collect the debt via a third-party collection service.
