We accept insurance and are pleased to be able to offer in-network services with most major insurance companies. In-network status varies by provider. We recommend that you consult your insurance company about potential pre-approval requirements and reimbursement issues prior to your appointment with us. Your insurance plan may cover all or part of the cost of services. Click here to download and print an evaluation referral form for your doctor to complete prior to your visit.
We are also happy to work with individuals who do not have insurance on a self-pay basis.
Co-pays, balances, and payments for non-covered services and products are due the day of service.
We accept:
A 24-hour notice is required for cancellation, except in emergency situations. We charge a $50 fee for late cancellations and no-shows.
It is the client's responsibility to consult their insurance company prior to the initial visit to determine coverage.
**Fees for service are determined by Complete Neuropsychology Services' administrative staff, who reserve the right to change rates and policies at any time. Further, if you do not pay for services as previously agreed, the clinic reserves the right to attempt to collect the debt through a third-party collection service.
Academic testing for learning disabilities in reading, writing, and/or math is not billable through insurance. Comprehensive academic testing is offered at a flat rate of $700.00, which includes assessment across all academic domains as clinically indicated.
If only select academic areas are required for diagnostic purposes, the cost will be prorated accordingly. Payment is due on the day of testing.
